I , hereby authorize ABC Services of Maryland, LLC, employmentr its agents tomake an independent investigation of my background, references, character, past employment, education, credit history, criminal or police records, including those maintained by both public and private organizations and all public records for the purpose of confirming the information contained on my application, and/or obtaining other information which may be material to my qualifications for employment now and, if applicable, during the tenure of my employment with the Company. I release ABC Services of Maryland, LLC, and/or its agents and any person or entity, which provides information pursuant to this authorization, from any and all liabilities, claims or law suits in regards to the information obtained from any and all of the above referenced sources used.I further understand that any information gathered in the Criminal Background Check, Motor Vehicle Report (MVR), and verification of listed references or drug screening test deemed unacceptable by the Main Office can result in immediate termination and that any information gathered in those same checks deemed marginal will be scrutinized by the Main Office and may result in termination or extended period of probation.

I grant ABC Services of Maryland, LLC, to deduct the sum of $35.00 from my first paycheck in order to conduct Criminal Background and MVR checks on my behalf.

I have been given a stand-alone, consumer notification that a credit report, not affecting my score, will be requested and used for the purpose of evaluating me for employment, promotion, reassignment or retention as an employee.
